Return to School Questionnaire
To help prevent the spread of Covid-19 in Killeshin N.S. and the local community, parents are asked to fill out this questionnaire. It must be completed shortly before returning to school after an extended closure. Also, if your child is absent due to illness for any period of time the survey must be completed the day before returning to the classroom.
